Choose Your Charity (CYC) Request Form

This CYC Request Form is to be used by fundraising participants or teams to request directed funding from the Pledge the Pink Foundation for breast cancer screening, treatment, outreach, and research programs that align with its mission. To help close gaps in care and reduce barriers to access, applicants should explain how the requested funds will support eligible services such as mammograms, biopsies, clinical trials, research, diagnostic equipment, or outreach and education initiatives.

CYC Eligibility

To be eligible, an individual or team must raise more than $5,000, and 75% of the amount raised may be directed to the board-approved charity.

CYC Timeline

Within 3 business days

The CYC contact will receive an email acknowledging that your Choose Your Charity (CYC) form has been received. Please check your spam or junk mail.

Within 5 business days

A Grant Form will be sent to your CYC Charity Contact with a copy to the CYC Requestor for completion.

Within 10 business days after submission

Once the completed Grant Form is received the Board will contact you should they have any questions or need further information or you will be provided with a date when the Board will review and approval.

Monthly Board Meetings

Board Meetings are held on the First Tuesday of Each Month. Grant forms received by the 15th of the prior month will be presented to the Board for Review

Decision Communicated

Decision communicated to CYC Requestor with copy to CYC Charity Contact. No amount will be communicated until after the event is final.

PTPF Board Review

All submissions will undergo formal Board review based on mission alignment, community impact and reach, demonstrated need, financial stewardship, organizational strength and stability, program sustainability, measurable outcomes and accountability, and support of PTP events. The Board will assess how well each proposal meets these criteria and demonstrates clear need, sound stewardship, sustainability, and meaningful impact.
